Applications require submission of a personal statement, academic transcripts, three recommendation letters, and a curriculum vitae all completed by January 5, 2018 through the online PhORCAS system. A PGY1 pharmacy residency is a pre-requisite and an on-site interview is required.
This program is accredited by the American Society of Health-System Pharmacists (ASHP). Also, you must be registered for The ASHP Residency Matching program. Please refer to the ASHP website for more information.
